Are you on a Canadian Electronic Medical Records (EMR) system and need an export of all your charts? EMR providers often charge hundreds of dollars per month indefinitely to keep the system running just so you can access your data - even after closing the practice.
Paying to download the raw database still leaves you with data you can't easily use, and no single PDF per patient - which is what you actually need when dealing with archived records.

How to get your data from your EMR
All EMR providers (Oscar, JUNO, Avaros, Accuro, Telus, PSS and others) that are or have been OntarioMD certified can export charts to the CDS format. That produces one XML file per patient, encoded in OntarioMD's Core Data Set Standard (CDS-S).
When you request a data export from your EMR vendor, ask for CDS format. You'll receive one XML file per patient containing everything in their chart - the same format vendors use when migrating between EMRs. Uploaded files such as scanned PDFs are also embedded (base64-encoded) inside the XML, so the CDS export contains everything available.
The EMR CDS-S represents a logical set of data elements that OntarioMD-certified EMR offerings support to display and persist. It provides a consistent method to describe data between disparate healthcare systems and the EHR products and services to which they interface.
ADITECH CDS XML to PDF
The program is called ADITECH CDS XML to PDF. It is currently in use by the largest nonprofit medical records storage company in BC.
Select the folder containing your XML files and press one button. PDFs are saved in the format: LASTNAME, FIRSTNAME PHN DOB.pdf
Regardless of the source EMR, once you have charts in CDS format this program will convert them all into per-patient PDFs. It scans the selected folder including subfolders and ZIP archives for any XML file. Older CMS Spec exports are also supported.
